605 Run Crew Waiver

    Welcome to the 605 Run Crew. We are excited to have you join our group run sessions. We want the best experience for all at our sessions and that means looking after ourselves, each other and being responsible for our actions and choices. Please complete the following form which details our safety and liability requirements. Once completed you will be able to register to attend any 605RC run session using the Teams App. Located here: https://www.teamapp.com/ or search for teams in the app store. Once you have the app you can search for 605RC and the calendar there will show you all our upcoming sessions.

    READ BEFORE SIGNING
    In consideration of being allowed to participate in any way in 605RC athletic sports sessions, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

    The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist. For recreational road and trail running these may include but not limited to;
    -UV exposure and heat illness: Sunscreen and skin protective clothing should be used to protect you.
    -Interactions with vehicles: When crossing any roads and/or thoroughfares always remain vigilant, give way, look both ways, cross only when clear and safe to do so.
    -Interactions with other path and trail users- Keep to the left, slow down, give way and move off the trail/path where safe to do so. When approaching path users from behind inform them of your intention to pass.
    -COVID-19 Transmission: It is your responsibility to keep up to date with changes to government COVID-19 rules and regulations. All information should be sourced directly from https://www.dhhs.vic.gov.au/coronavirus and should be reviewed regularly to ensure currency and validity of information. All directives apply to our sessions. These directions include but not limited to;
    1. Practise good hygiene and regularly and thoroughly wash your hands.
    2. Maintain physical distancing, keep at least 1.5 metres away from others.
    3. All Victorians must carry an approved mask/face covering, to be used when not running and within 1.5m of others.
    4. If you feel unwell do not attend a session.
    5. If you do feel unwell and have attended a session within the past 2 weeks, report it immediately to the session run leaders.
    6. Declare any COVID-19 symptoms that may arise during a session to the session run leader, isolate yourself and do not interact with any other session attendees.
    7. Only attend sessions you have registered for and have supplied your contact tracing information
    8. Avoid sharing personal belongings
    9. Do not attend any session where there is already 50 attendees
    -Low light: During hours of darkness and low light, you should have a suitable, fit for purpose running headlamp or torch.
    -Snake and Insect bites: Declare all allergies to the run leaders and carry your medications. Trail runners should ensure a snake bandage is accessible and a phone to coordinate emergency response.
    -Uneven and slippery surfaces: Injuries can result from trips and falls. All care should be taken when running, with constant attention to the conditions around you and your path of travel. Suitable and fit for purpose running footwear should be worn. Session attendees should self pace and run to their abilities and to the conditions.
    -Trail running has hazards including low hanging branches and sharps. Protective clothing should be considered to prevent puncture and scratch injuries to body and face.
    -Dehydration: Session attendees should ensure they are suitably hydrated before their session and ensure they have water available during it.
    -Security of personal belongings: Whilst personal belongings may be left with run leaders or in attendees vehicles, it is still possible theft may occur. You leave your personal belongings here at your own risk.
    -Before commencing vigorous activity after a period of inactivity, you should consult your GP to ensure the intended physical activity is suitable for your current physical and mental condition.
    -Mental Health Injuries. All session attendees are expected to;
    1. Respect the rights, dignity and worth of all others;
    2. Act with honesty and integrity;
    3. Treat others in a fair, ethical and considerate way;
    4. Respect the law and act in accordance with it;
    5. Respect at all times the enjoyment, safety and welfare of all Participants;
    6. Communicate and interact with ALL others in a way that is respectful and inclusive
    and,
    I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
    I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
    I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS 605RC their coaches, run group leaders, officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
    I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. ANY BREACHES OF THE ABOVE ARRANGEMENT MAY RESULT IN REFUSAL OF FUTURE SESSION ATTENDANCE.
